本剤による重篤な肺炎発生が問題化しているニュースというので調べてみた[これは有料購読者でないと本文が読めないので]。 本剤服用時に肺炎発生率がなぜか多くなる、という点は、承認以前に問題視され、添付文書にも「注意事項として」明記されている。
予想以上に深刻化したのか？ しかし詳細な情報はまだ得られていない。次の注射時反応によるFuzeon投薬中止についても、Pathology of Injection Site Reactions with Fuzeon[2003.2.14]にあるように目新しい問題ではないのだが．．．

日本に関しては、エイズ動向委員会報告によると、(2007.12.末現在)HIV感染者9,392人、エイズ患者4,450人、凝固因子製剤による感染者1,438人 合計15,280人 死亡251人。 新規患者は2007年間でHIV 1048,エイズ400人。
抗HIV薬は最もドラッグラグの少ない領域で、欧米で承認された新薬の大半が日本でも優先的に承認されてきた。
薬物療法の標準は作用機序の異なる抗HIV-1薬を併用する多剤併用療法(HAART;Highly Active Anti-retroviral Therapy)であり、実態調査によると、服薬率95%以上でのHAART治療成功率は78.3%と比較的高いが、一方副作用頻度の合計は34～59％と高率であり、軽微な副作用も含めると「どの組み合わせを選んでも何らかの副作用が生じる」のが問題。 更に多剤耐性変異株の出現と慢性毒性の蓄積が課題として残る。 そのため優れた新薬の出現は歓迎される

世界のHIV/AIDS患者数は、2005年末推定3860万人。 毎年410万人の新規感染者が発生。 死亡者数は1998年末までの累計は1390万人、2001年では300万人、2002年310万人、2005年280万人。 日本に関しては、エイズ動向委員会報告によると、(2006.10.1現在)HIV感染者8,091人、エイズ患者3,949人、凝固因子製剤による感染者1,438人 合計13,458人 抗HIV薬は多数開発されてきたが、進行を遅らせるにとどまっている。1986年には作用機序の異なる抗HIV-1薬を併用する多剤併用療法(HAART;Highly Active Anti-retroviral Therapy)の有効性が報告され, 一時は治癒可能ではないかとまで期待された。 しかし, 長期にわたり残存する感染細胞が証明され, HAARTを中断すると必ずウイルスの再増殖が見られることから, 現在では生涯治療が必要であると考えられている。 このようにHAARTは「長期継続」が必要な治療法であるが, 長期継続に伴うさまざまな問題点が明らかとなってきた。 1つは薬剤耐性変異株の出現であり, もう1つは慢性毒性の蓄積である。

新たなプロテアーゼ阻害剤には、(1)高い経口吸収性 (2)長い血中半減期 (3)低い蛋白結合率 (4)交叉耐性のない薬剤耐性 プロフィルが求められている。
今回採りあげたプロテアーゼ阻害剤ダルナビルは、イリノイ大学ゴーシュ教授が合成し、熊本大学満屋裕明教授が生体内での効果を確認、共同開発したもので野生株および多剤耐性のウイルス株にも高い活性を示した。
プロテアーゼはＨＩＶに含まれる酵素で、ＨＩＶの増殖に必要なタンパク質を正しく切断する「はさみ」の役割を果たす。 ＰＩは酵素に付着してはさみを切れなくし、ＨＩＶの増殖を止める。 既存のＰＩは、酵素の成分であるアミノ酸の端に結合する性質があり、ＨＩＶの遺伝子が変異してアミノ酸が変わると効かなくなってしまう弱点があった。
これに対しダルナビルは、アミノ酸が変わっても影響ない場所に結合するため耐性ができにくい。 ＰＩなどに耐性が検出された患者への臨床試験では約７０％に治療効果がみられ、別の薬の3倍以上高かった。
エイズ動向委員会報告によると、(2005.4.3現在)HIV感染者6,734人、エイズ患者3,336人、凝固因子製剤による感染者1,434人 累計11,504人。 日本での感染は依然抑止されており、抗エイズ薬市場としての日本は小規模。
国際新薬は、優先的に迅速審査され日本にも導入され、1996-2000年の５年間に１５新薬が日本で承認された。 その後３年間の空白があり硫酸アタザナビル(レイアタッツ[BMS],2003.12承認)、フマル酸テノホビル ジソプロキシル(ビリアード[日本たばこ－鳥居],2004.3.25承認,2004.4.12発売)、エムトリシタビン(エムトリバ([日本たばこ－鳥居],2005.3.22承認,2005.4.19発売)、そして今回採りあげるツルバダ(TM)錠([日本たばこ-鳥居]2005.3.22承認,2005.4.19発売,エムトリシタビン・フマル酸テノホビル ジソプロキシル配合剤)と立て続けに承認販売され、国際導入新薬は一通り日本でも揃った。

エイズ治療はこの10 年間ほどで急速な進歩をとげた。 HIV遺伝子の働きを抑えて、未感染の細胞に感染しにくくする逆転写酵素阻害剤（reverse transcriptase inhibitor,RTI ）２種とHIV感染細胞からのHIV産生を抑える優れた蛋白分解酵素（プロテアーゼ）阻害剤（protease inhibitor, PI）（あるいは非ヌクレオシド系逆転写酵素阻害剤）1 種との組み合わせによる多剤（3 剤）併用療法（highly active antiretroviral therapy, HAART ）が奏効し、米国では死亡数が半減した。
しかし耐性の問題などから死亡数減少も頭打ちになりつつあり、副作用の問題も依然残されている。（HIV感染症の臨床症状、予防法などについて[2003.12.11 by 愛知県衛生研究所]が最新の優れたレビュー）1069:日本語版註) HAART: 抗HIV療法と服薬援助のツール[中四国エイズセンター：資料,1998] ;多剤併用（カクテル）療法などの強力な抗ウイルス療法のことで、96年に米国で打ち出された。日本では97年から本格的に行われるようになり、HIV感染者の入院減少、死亡数の減少がみられている。

全く新しい融合阻害剤という種類の抗HIV治療薬が米国で発売された。 画期的新薬で副作用の点でも、臨床試験では98%の患者で注射部位反応が観察されたが、投薬中止は3%と少なかった。
どういう点が画期的かというと、他の抗HIV薬が、既にヒト免疫細胞(CD4)に侵入している状態のHIVに対して働くのと違って、Fuzeonは最初の段階で、HIVが免疫細胞に感染する能力をブロックするわけだ。
今年３月末に発売されたが、供給体制の問題もあり、2003年Q2売上はも米国$4.3 million、米国外$870,000。【1069:日本語版コメント】

世界のHIV/AIDS患者数は、2005年末推定3860万人。 毎年410万人の新規感染者が発生。 死亡者数は1998年末までの累計は1390万人、2001年では300万人、2002年310万人、2005年280万人。[●WHO Report on the global AIDS epidemic - May 2006] このうち87%がAA諸国で占めるとはいえ、欧米先進国でも少ないわけではない。 米国でHIV感染者は95万人、毎年4万人が感染。 2004年度末現在で生存するエイズ患者数110万人、累積死亡者529,113。[[CDC]HIV/AIDS Prevention :Statistics and Surveillance]

日本に関しては、エイズ動向委員会報告によると、(2007.12.末現在)HIV感染者9,392人、エイズ患者4,450人、凝固因子製剤による感染者1,438人 合計15,280人 死亡251人。 新規患者は2007年間でHIV 1048,エイズ400人。

新たなプロテアーゼ阻害剤には、(1)高い経口吸収性 (2)長い血中半減期 (3)低い蛋白結合率 (4)交叉耐性のない薬剤耐性 プロフィルが求められている。

・As of the end of 2001, an estimated 40 million people worldwide ? 37.2 million adults and 2.7 million children younger than 15 years ? were living with HIV/AIDS. More than 70 percent of these people (28.1 million) live in Sub-Saharan Africa; another 15 percent (6.1 million) live in South and Southeast Asia.(1)・Worldwide, approximately one in every 100 adults aged 15 to 49 is HIV-infected. Has Trizivir taken off?
GlaxoSmithKline launched its new triple combination HIV therapy Trizivir at the end of 2000. The tablet contains no new molecules --- its novelty lies in the combining of three active ingredients, reducing the number of pills needed to be taken each day and therefore increasing patient convenience and compliance.
Trizivir contains the two mainstay HIV drugs Epivir (lamivudine, 3TC) and Retrovir (zidovudine, AZT) with Ziagen (abacavir). GSK had previously launched Combivir, which combines lamivudine (licensed from Shire's BioChem Pharma subsidiary) and zidovudine. MIDAS , the viewing platform for IMS HEALTH data, shows that Combivir is now the leading drug in the J5C HIV antiviral therapy class.
Top ten HIV drug sales
Source: MIDASTrizivir is taken twice a day, and there are no restrictions regarding food or water intake. Despite these benefits, its sales have so far been modest, amounting to $77.4 million from launch through the end of June 2001, according to MIDAS. But, it was only approved in the EU in January 2001, and sales did almost double from the first to the second quarter of the year.
GSK dominates market
If nothing else, Trizivir will help consolidate GSK's position as the leading HIV specialist. Bristol-Myers Squibb follows, strengthening its own position through the acquisition of DuPont Pharmaceuticals, which makes the popular HIV drug Sustiva (efavirenz), a non-nucleoside reverse transcriptase inhibitor (NNRTI). Sustiva is one of the few members of the J5C class that is seeing significant growth, the other being Abbott's protease inhibitor Kaletra (lopinavir/ritonavir).

The need for new treatments was made clear in November 2001, when the Proceedings of the National Academy of Sciences published a paper detailing the discovery of a new class of HIV that has the ability to replicate and mutate rapidly, increasing its chances of becoming resistant to zidovudine --- the cornerstone of most HIV treatment regimens. If an HIV strain developed multi-drug resistance, most current products would be powerless against it. Yet ironically, the success of current regimens means that fewer patients are volunteering for clinical trials of new drugs.
Promising findings at ECCATHI
Results from a number of studies with existing and future treatments were presented at the 8th European Conference on Clinical Aspects and Treatment of HIV Infections (ECCATHI), held in Athens at the end of October 2001.
- Sustiva
BMS presented data from a 1,078-patient study suggesting that Sustiva had greater potency and durability in treatment-naïve patients than Boehringer Ingelheim's Viramune (nevirapine). Two smaller trials demonstrated that when patients swapped their protease inhibitor (PI) for Sustiva, viral load suppression was maintained for a longer time than in patients continuing with the PI. Moreover, a combination of Sustiva, Videx (didanosine) and the investigational nucleoside reverse transcriptase inhibitor emtricitabine (Coviracil, FTC - Triangle/Abbott) taken once daily showed strong antiviral and immunological efforts over the course of a 96-week pilot study.
- Atazanavir
BMS also published findings on its "novel and potent" PI, atazanavir, currently in Phase III trials. When given as part of a HAART (highly active antiretroviral therapy) regimen, atazanavir was associated with lower total cholesterol, LDL and triglyceride levels than a similar regimen containing the PI nelfinavir (Pfizer's Viracept). Total cholesterol levels rose 5-6% with atazanavir, compared to 25% with nelfinavir.
- Viramune
Boehringer Ingelheim published the findings of two studies regarding its NNRTI, similarly demonstrating that it was associated with a favourable lipid profile, unlike many PIs. At 96 weeks, patients in the Viramune arm of the FRAMS study saw a 40% increase in their HDL cholesterol levels, and a decrease in total/HDL cholesterol ratio, when compared to patients taking indinavir (Merck & Co's Crixivan).
Other study data released
At an earlier conference in Argentina, Pfizer's Agouron subsidiary presented its own findings on Viracept --- a retrospective chart review of 1,039 patients showed that when used as part of an initial HAART regimen, Viracept provided durable and potent viral load suppression, plus an increase in CD4 cells.
Likewise, at a US meeting, Roche published data on its PI Fortovase (saquinavir). A retrospective review of 23 patients with HIV and hepatitis C virus infection receiving methadone (for heroin addiction) indicated that once-daily Fortovase plus a "mini-dose" of Abbott's Norvir (ritonavir) could help simplify the HIV treatment schedule.
Setbacks and optimism
There have been some disappointments in the HIV antiviral arena in 2001. In January, Agouron announced that it was restricting the use of its experimental NNRTI capravirine in clinical trials due to the development of vasculitis (inflammation of the blood vessels) in an animal toxicology study. The adverse effect was seen at very high doses, and had not occurred in any patients, so Pfizer said it was discussing the data with the FDA.
In December 1999, the FDA asked for more data on Triangle's Coactinon (emivirine), another NNRTI originally developed by Mitsubishi, and included in Triangle's 1999 strategic alliance for antivirals with Abbott.
In October 2001, however, the FDA approved Gilead Sciences' nucleotide analogue reverse transcriptase inhibitor Viread (tenofovir disoproxil), the first in its class. It is taken once daily in conjunction with other antiretroviral agents. Market leader GSK and Vertex are running late Phase III trials with a new PI, fosamprenavir (GW 433908), a pro-drug of their Agenerase (amprenavir); it is scheduled for filing in 2002 and is said to have a better side effect profile than Agenerase.
Also in 2002, Roche and Trimeris are hoping to file T-20 (pentafuside) for approval. This is from a new class of drugs known as fusion inhibitors, which stop HIV entering host cells. Due to its novel mechanism of action T-20 and its follow-up compound T-1249 should help combat HIV's resistance to current therapies. Positive data were presented at ECCATHI. The fusion inhibitors, however, are costly and need to be injected, so they are unlikely to be used as first-line treatments.
Although growth has slowed in the HIV market, this is not surprising given the phenomenal expansion between 1995 and 2000. IMS HEALTH's Pharmacast &Beyond forecasting service predicts 2.32% annual growth in the J5C class, from $2.6 billion in 2000, to $3.3 billion in 2010, driven by the launch of new protease inhibitors, fixed combination drugs such as Trizivir, and the fusion inhibitors.See Also: AIDS: Someone else's problem...? 種類の関連レトロウイルス（HIV-1およびHIV-2）のいずれかによって引き起こされる感染症で，無症状の保菌者状態から細胞性免疫の不全に関連した重篤な消耗性の致死性疾患まで，広範囲の臨床症状を起こす。レトロウイルスは発癌性のものもあれば，正常な細胞機能を変えたり細胞死を引き起こす病理的作用をもつものもある。人間に感染することが知られているレトロウイルスのうち，HTLV（ヒトT細胞ウイルス）I 型およびII型はリンパ性悪性腫瘍と神経疾患に関連しており，重度の免疫抑制との関連性はそれより低い。一方，HIV（ヒト免疫不全ウイルス，エイズウイルス）は免疫抑制を引き起こすが，腫瘍は直接的には引き起こさないようである。
HTLV-I とHTLV-II は両方ともリンパ球向性で発癌性のC型レトロウイルスで，感染者の5％未満で成人T細胞白血病/リンパ腫を引き起こす。組織および血中におけるCD4＋T（ヘルパー）リンパ球の拡大は，白血病，びまん性リンパ節腫脹，肝脾腫大，および皮膚病変を引き起こす。多くの患者が免疫抑制状態を呈し，一部の患者は進行したHIV感染の場合と同じ日和見感染症を起こしやすい。HTLV-I も神経向性で，保菌者の1％未満において進行性脊髄症（熱帯性痙性不全対麻痺またはHAM［ヒトT細胞ウイルスI型脊髄症］）を引き起こす。HTLV-II 保菌者では，数例の脊髄症が報告されている。臨床的にみて，HAMは脚の衰弱，硬直，しびれ感，知覚不全，そして頻尿や失禁が感染後の最初の10年以内に出てくる進行性痙性不全対麻痺である。
HTLV-I は性的接触や血液を介して伝染するが，ほとんどの感染は授乳によって母親から子供へと垂直に伝播されているようである。HTLV-I の疾患と抗体保有率のパターンは，このウイルスの分布が広範囲であるが，一様ではないことを示している。例えば，高レベルのHTLV-I が日本の南部とカリブ海地域にみられ，また米国の一部の都市の静注薬物乱用者や売春婦にもみられる。
これまでに最も大きな社会的かつ医学的影響を及ぼしたヒトレトロウイルスはHIV-1で，これは1984年にAIDS（後天性免疫不全症候群）と呼ばれる重度免疫抑制の広域集団発生の原因として同定された。
AIDSは細胞性免疫の障害で，日和見感染症，悪性疾患，神経学的機能不全など多彩な症候群を特徴とする。AIDSはHIVに関連した一連の疾患のうち，最も重篤な症状発現である（後述の症状と徴候の項参照）。未治療のHIV感染者がAIDSを発症する危険性は，感染後最初の数年は1～2％/年で，それ以降は5％/年と見積もられている。積算リスクは最初の10年間で約50％である。未治療のHIV感染者のほぼ全員が，いずれはAIDSを発病する。HIV感染の長期感染の結果（例，他の悪性疾患や慢性神経疾患）の中には，まだ十分に明らかにされていないものもある。
●分類
AIDSは最初，重篤な日和見感染症および/または不完全な細胞性免疫に関連していることが判明しているカポジ肉腫や非ホジキンリンパ腫といった特定の続発性癌の発現によって定義された。米国CDCの1993年の定義は，青年と成人を無症候性（A），HIVに起因する疾患を伴う症候性（B），および真性AIDS（C）に分類している。HIV患者はまた，CD4＋リンパ球数によっても分類される：500/μL 以上（1），200～499/μL（2），200/μL未満（3）。多くの患者が，慢性症状を事前に経験することなく，生命を脅かす日和見感染症あるいは悪性疾患と診断されることによって初めてHIV感染に気がつく。

●疫学
AIDSは1981年に最初に発見され，カリニ肺炎とカポジ肉腫の症例が米国カリフォルニア州とニューヨーク州の同性愛男性で報告されたが，それ以来1996年12月までに，米国で581,000件以上の症例と357,000件の死亡が報告された。全世界では，3,000万を超える数のHIV感染者と1,000万のAIDS症例があると見積もられている。 HIV伝播には2つの疫学的パターンが確認されている。米国とヨーロッパ（タイプ1）では，主として同性愛または血液を介して伝播する。ほとんどの患者がハイリスク群（例，同性愛または両性愛の男性，注射針を共用する静注薬物常用者，輸血または血液製剤の投与を受けた人でときに女性へのHIVの異性的伝播を起こす人）に属する20～49歳の男性である。米国では，AIDSの全症例に占める女性の割合が増えている（約20％）。血友病および他の出血性疾患患者の間で，AIDSは死因のトップになった。1985年以前は，血友病患者でのHIV感染の危険性は，第Ⅷ因子製剤に対する需要が大きいことと，米国内にある血漿製剤の起源に関連していた。米国が起源の市販用血漿製剤が広く出回ったことにより，当初は集団発生の影響を受けていなかった地域の血友病患者の間ですらHIV感染率が高まる結果となった。ヨーロッパの大部分の地域では，HIV感染リスクの低い人々から凝固因子が集められていたため，感染した血友病患者の数は少なかった。しかし，スクリーニングならびに熱処理をした血液や，生物工学による治療薬が血友病に対して日常的に使われるようになったことにより，その後は感染の危険性はなくなった。
アフリカ，南米，南アジア（タイプ2）では，伝播は主として異性間である。これらの地域では，男性と女性がほぼ同率で感染している。ブラジルやタイなどの国では，2つのパターンの混合が見つかっている。概して，疾病は交通経路に沿ってまず都市に広がり，それから二次的に地方へ広がる。 集団発生への対処に必要な資源が少ない発展途上国でHIVが広がり続けていることは，危機をはらんだ事態である。タイで2種類の異なる血清型のHIV-1が広がっていることがそれを例証している。1990年頃には，異性感染（遺伝子型A）HIVと注射針感染（遺伝子型B）HIVの流行が並行して，女性の売春婦と売春相手，そして注射針を共有するアヘン中毒者の間に急速に広がった。
出産適齢期の女性が多数感染したことは，相当数の小児AIDS症例を生みだした。HIVは経胎盤的に，あるいは周産期に伝播されることがある。母乳にウイルスが見つかっており，授乳が伝播に関与している。加えて，十分に減菌されていない針を繰り返し使用することで新生児および小児が感染したケースが多数ある。
●症状
HIVは他の疾患と同様，様々な臨床的問題を引き起こす。感染直後とその後しばらくの間（少数の人で数カ月以上），短期間の抗体陰性保菌者状態がある。この間，ウイルスは免疫機構が反応し始めるか，それに加えてターゲットが消耗するまで繁殖する。HIV RNAまたはHIV p24（カプシド）抗原は，HIVに対する抗体が検出されないときにも血漿から検出できる。感染後1～4週間以内に，一部の患者は発熱，倦怠感，発疹，関節痛，全身のリンパ節腫脹を伴う急性レトロウイルス症候群または初期HIV感染症を発現し，それが通常3～14日続いた後，数日から3カ月以内にHIV抗体が陽性になる。急性レトロウイルス症候群は，発熱性の上部呼吸器疾患（いわゆる流感）あるいは単核球増加症としばしば誤診される。その後，これらの急性症状は消え（だがリンパ節腫脹は続くことが多い），患者は抗体陽性無症候性HIVキャリアとなる。これらの患者の中には，AIDSの定義に合致しない軽度で間欠性の症状や徴候を示す患者もいる（例，鵞口瘡，帯状疱疹，下痢，疲労，発熱）。白血球減少がよくみられ，貧血と免疫媒介性の血小板減少が起こることもある。 神経症状：神経症状はよくみられ，AIDSの初発症状であることも多い。症状はHIVの直接的な作用，日和見感染症，新生物，あるいは血管の合併症が原因であろう。症状としては，急性無菌性髄膜炎；数種類の型の末梢神経障害；けいれんを伴う脳障害；局所性の運動，感覚，または歩行障害；および痴呆に進行する認識機能不全がある。末梢神経障害は有痛性の感覚異常，中等度の遠位感覚消失（靴下-手袋状），アキレス腱反射の低下，遠位部筋力低下，および萎縮を引き起こすことがあり，様々な程度で起こる。ギラン-バレー症候群またはCMV（サイトメガロウイルス）多発神経根障害のいずれかが上行性麻痺としてみられることがある。 多発性筋炎に似たミオパシーがAIDSやジドブジン治療時に合併することがある。
無菌性髄膜炎は頭痛，発熱，および羞明を引き起こすことがあり，髄液単核球増加症に関連している可能性がある。一過性の無菌性髄膜炎が一次性のHIV感染に伴うことがある。
亜急性脳炎がHIV，CMV，または両方によって引き起こされることがある。剖検時の神経病理所見が，他の炎症性浸潤を伴わないミクログリア細胞の結節状集合が灰白質にあるのを示すことがある。CMV脳炎の場合は，CMVの核内および細胞質内封入体が結節に関連している。明確に定義されていない小さな細静脈周囲の脱ミエリン化部位が白質内にみられる。頭痛，錯乱，記憶喪失，精神運動の減退，ミオクローヌス，けいれん，そして昏睡に進行する重篤な痴呆が，死に至る前の数週間から数カ月にわたる典型的な所見である。CT上での皮質の萎縮，髄液の細胞増加と蛋白レベルの上昇，そして広範な異常脳波がみられることがあるが，特異的ではない。ポリメラーゼ連鎖反応によって髄液に対するCMV-DNAを実証することが，CMV脳炎，脳室炎，または脊髄炎/多発神経根障害の診断確定になる。
多くのAIDS患者で軽度の認識と運動障害が起こるが，臨床的にも社会的問題もはっきりそれとわかる程度ではなく，広くは認識されていない。侵されることが多い認識機能は，注意力，情報処理速度，および学習の分野である。これらの認識異常は，気分の障害，あるいは薬物やアルコール乱用では説明できないものである。これらは，MRI上での脳萎縮，免疫活性化（β2ミクログロブリンレベルの上昇），脳脊髄液における測定可能なHIV RNAレベル（200コピー/mL以上），および他の神経異常に関連している。軽度の認識および運動障害は必ずしも急速には痴呆に進行しないが，多くの患者でゆっくりと進行する荒廃がみられる。CMVまたはHIV脳症の治療に対して反応があることが報告されているが，予測できない。
中枢神経系の日和見感染症：トキソプラズマ脳炎は頭痛，傾眠，錯乱，けいれん，および局所徴候を引き起こし，数日から数週にわたって続く。CTまたはMRI所見としては，基底核に好発するリング状造影がある。前駆的な慢性潜伏性感染症を反映して，IgG抗トキソプラズマ抗体の血清検査はほとんどいつも陽性であるが，必ずしもトキソプラズマ原虫による病変であるという証拠になるわけではない。血清検査が陰性なら，病変がトキソプラズマ原虫によって引き起こされたものである可能性は大幅に減少する。髄液は軽度から中等度の細胞増加と蛋白量の上昇を示す。脳生検が診断確定になる。しかし，ピリメタミンやスルファジアジン（あるいは患者がスルファジアジンに対してアレルギー性の場合にはクリンダマイシン）を用いた臨床試験が，7～10日間の反応を綿密に観察しながらしばしば血清陽性患者で試みられている。治療により予後は良好で，予防としてトリメトプリム/スルファメトキサゾールまたはクリンダマイシン/ピリメタミンを終生服用することにより再発を防ぐことが可能である。
AIDSではクリプトコッカス，ヒストプラスマ，および結核菌（ヒト型結核菌）による髄膜炎も発熱と頭痛を伴って現れ，治療可能である。パポバウイルスによって引き起こされる脳炎である進行性多巣性白質脳症（162章参照）は治療に反応せず，通常進行性で数カ月以内に死に至る。
脳の新生物：脳の原発性B細胞中枢神経系リンパ腫（非ホジキン）は，その解剖学的位置に一致した限局性徴候を引き起こす。CTは通常，ときに造影剤による強調が強く，トキソプラズマ，結核，クリプトコッカス，あるいは他の日和見生物によって引き起こされた限局性脳炎とは必ずしも区別できない腫瘤を示す。このような場合はMRIのほうが識別力が高く，確定的な診断には脳生検が必要である。AIDSにおける全身性リンパ腫は中枢神経系も侵すことがあるが，カポジ肉腫はめったに中枢神経系を侵さない。
血液症状：一部の患者は症候性貧血または免疫性の血小板減少症を示す。HIV関連の血小板減少症は，特発性血小板減少性紫斑病と同じ治療（コルチコステロイド，脾摘出，静注免疫グロブリン）に通常は反応するが，めったに出血には至らない。
消化器症状：腹痛，悪心と嘔吐，あるいは下痢が，進行したAIDS患者によくみられる体重減少とるいそう（やせ）を引き起こす。胃腸管を巻き込んだ様々な日和見感染症や腫瘍が起こる。病変が起こる部位は，口腔咽頭部（カンジダ，カポジ肉腫，リンパ腫瘍，単純ヘルペス，アフタ性口内炎），食道（単純ヘルペス，CMV，カンジダ），胃（カポジ肉腫，リンパ腫），腸（サルモネラ，クロストリジウム-ディフィシル，CMV，単純ヘルペスウイルス），および胆管（クリプトスポリジウム，CMV）である。さらに，薬剤性の膵炎（例，ジダノシン）または肝炎（例，フルコナゾール）が治療を複雑化することがある。重度の免疫抑制や他の症状がない患者でも，原因不明の下痢が長期間続いたり，間欠的に再発することがある。
皮膚症状：HIV感染の皮膚症状は，初期感染時の発疹や性器潰瘍から，AIDS発症後に広範囲にみられるカポジ肉腫まで，すべての段階を複雑化する。感染経過全般にわたってよくみられる帯状疱疹は，しばしば最初の症状である。クリプトコッカス症または細菌性血管腫症の血行性病変は，これらの日和見感染症の診断に重要な手がかりとなる。
口腔症状：口腔カンジダ症（鵞口瘡）はHIV感染における最初で最もよくみられる症状の1つである。通常は痛みがなく，患者自身が気づいていないこともあり，診断がついていない患者における有用な手がかりとなるものである。口腔毛状白斑は，舌の両側にみられる無症候性の拡大した白い糸状斑によって診断され，おそらくはEBウイルスによって引き起こされたもので，アシクロビルで治療可能である。単純ヘルペスによって引き起こされた潰瘍または病因不明の潰瘍（アフタ性）は，大きく痛みを伴い持続性で，栄養吸収を妨げることがある。歯周疾患が重篤になり，出血，歯肉の腫脹，歯の喪失が生じることがある。カポジ肉腫とリンパ腫の両方が口腔咽頭部にみられることがあり，通常は無痛の腫瘤である。
肺症状：最も重要なHIV関連の肺感染は結核で，結核が蔓延している地域では，しばしば最初のHIV感染の症状となる。非定型的症状（まれな空洞化，下葉浸潤，粟粒疾患，リンパ節腫脹），ツベルクリン皮膚試験に対するアネルギー，そして他の日和見感染症との混同または共存が診断を難しくすることがある。肺はまた，ニューモシスチス-カリニ，クリプトコッカス-ネオフォルマンス，ヒストプラスマ-ネオフォルマンス，コクシジオイデス-イミチスおよびアスペルギルス属などの真菌が原因の日和見感染症がよく起こる部位でもある。肺炎球菌，ヘモフィルス，シュードモナスおよびロドコッカスによって引き起こされる細菌性肺炎は，静注薬物乱用者で特によくみられる。カポジ肉腫とB細胞リンパ腫の両方が縦隔節と肺に生じることがある。
女性における症状：女性のHIV感染の症状と経過は，慢性難治性腟カンジダ症と子宮頸部上皮内癌の危険性が高い点を除いて，全体的に男性と似ている。骨盤炎症性疾患など一部のSTDは，HIV感染女性では非定型で侵襲性が高く，治療に対して抵抗性を示すことがある。再発性，侵襲性，あるいは異常に治療抵抗性のSTDまたは腟カンジダ症がある女性に対しては，HIV検査が推奨される。
心臓血管の合併症としては，衰弱性（血栓性）または細菌性心内膜炎（特に静注薬物乱用者），あるいはうっ血性心不全を伴う心筋症がある。
腎不全またはネフローゼ症候群がAIDSに併発することはまれだが，重篤な障害の原因になることがある。
●原因
●伝播HIVの伝播には感染細胞や血漿を含む体液との接触が必要である。HIVは，血漿やリンパ球を含む全ての体液や滲出液，正確には血液，精液，腟分泌物，母乳，唾液，傷からの滲出液の中に存在する。理論的には可能であるものの，唾液あるいは咳やくしゃみの際の飛沫小滴による伝播は極度にまれである。HIVは，ちょっとした接触は勿論のこと，職場，学校，あるいは家庭での非性的な近接接触でも伝播されない。最も一般的な伝播経路は，汚染された針の共有あるいは性的関係を介した体液の直接移動である。
●発生病理
密接に関連した2種類のレトロウイルス，HIV-1およびHIV-2が，異なる地理的地域でAIDSを引き起こしていると同定された。HIV-1は，西半球，ヨーロッパ，アジア，アフリカ中央部，南部，東部におけるAIDSのほとんどの症例を引き起こしている。HIV-2はアフリカ西部におけるAIDSの主因で，HIV-1より病原性は低いようである。西アフリカの一部の地域では，両方のウイルスが優勢である。
全てのレトロウイルスは，ウイルスRNAをプロウイルスDNAコピーに変換する逆転写酵素と呼ばれる酵素を持ち，このプロウイルスDNAが宿主細胞のDNAに組み込まれる。組み込まれたプロウイルスは，細胞が分裂するたびに正常な細胞の遺伝子によって複製される。このようにして，最初に感染した細胞の全子孫がレトロウイルスDNAをもつことになる。プロウイルスHIVのDNAはRNAに転写され，蛋白に翻訳されて，数百コピーの感染性ウイルスを産生する。HIVのライフサイクルの最終段階で重要なのが，もう1つの酵素，HIVプロテアーゼである。この酵素は，感染ヒト細胞から出芽後にウイルス内で再配列できるよう，重要な蛋白を分割することにより，未熟な非感染性HIVを感染型に変換する。
HIVは，表現型がT4すなわちCD4の膜貫通性糖蛋白をもつことにより，機能的にはヘルパー/インデューサー細胞として定義される主要なTリンパ球サブセットに感染する。HIVはまた，マクロファージ，ミクログリア細胞，種々の内皮細胞や上皮細胞などの非リンパ系細胞にも感染する。リンパ節の樹状細胞は細胞表面でHIVを結合できるが，侵入されない。HIV感染の結果として，T細胞，B細胞，ナチュラルキラー細胞，単球-マクロファージの数や機能が障害される。CD4＋リンパ球以外の細胞にも異常があるにもかかわらず，AIDSにおける免疫機能不全の多くは，これらのリンパ球から細胞性免疫に不可欠なヘルパー機能が欠損していることによって説明される。
●診断
●検査
HIVに対する抗体の検出は，感染のほとんどの段階において感受性が高く特異的で，また安価で広く利用可能である。迅速（10分間）血清検査，自宅収集システム，および口腔分泌物のHIV抗体検査は状況によっては有用だが，標準の血清検査による確認を要する。血液中のHIVDNAの検出は，抗体がまだ検出可能になっていない非常に初期の感染段階でHIV感染を高感度かつ特異的に診断できる。HIV抗体を検出する検査の1つがELISAで，HIV蛋白に対する抗体を検出できる。ELISAは感受性と特異性が両方とも高いが，検査結果が偽陽性になることがある。反応があればELISAを同一サンプルで繰り返すべきである。2回目も陽性になった場合には，例えば，免疫電気泳動の技法により，分子量によって分けられた特異的ウイルス蛋白に対する抗体を同定するウエスタンブロットなど，さらに特異的な検査を行うべきである。
抗ウイルス抗体ではなく抗原（p24）を直接測定するELISAは，比較的感受性が低い。抗原濃度の検査に代わって，より感度の高い血漿RNAの測定が使われるようになっている。
ウイルスの核酸を増幅するRT-PCR（逆転写酵素ポリメラーゼ連鎖反応）やシグナルを増幅するbDNA（分枝DNA）など数種類の高感度血漿RNA定量法は，広範囲のウイルス濃度（血漿1mL当たり100万コピーまで）にわたって感受性が高く正確である。検出の下限はRT-PCRで約400コピー/mL，bDNAで5000コピー/mLであり，これらの検査の感度は向上している。HIV RNA定量化の感度を上げるために，NASBA（核酸配列ベース増幅）やTMA（転写媒介性増幅）といったような他の核酸増幅法が開発中である。
●治療
1990年代中頃に，HIV感染に対するいくつかの新しい治療方針が出現した。血液中のHIVに対する薬の効果，つまり血漿HIV RNA量の抑制を迅速に測定する新しい方法が開発され，臨床的に不活発な感染段階でもHIVが急速に産生される点に関する解明が進んだことにより，アプローチが変わったのである。通常，2つの酵素（HIV逆転写酵素とプロテアーゼ）をターゲットにする薬物併用が現在では標準となっており，単独の薬物投与は奨励されていない。2～4種類の薬を用いて治療することにより，ウイルスの増殖を速やかに止め，免疫機能を保ち，ウイルスの薬剤耐性突然変異株が出現する可能性を減らせる。様々な薬の組み合わせに対する反応の長さは，ウイルスの複製を完全に抑制できたかどうかによって異なり，これには通常，3種類の強力な薬の併用を徹底して遵守することが要求される。
血漿HIV RNA量は，抗レトロウイルス薬の効果を迅速かつ直接的に測定する手段になる。RNA濃度を治療的にモニタリングすることにより，薬物併用療法の初回効果（4～8週目）と進行中効果（3～4カ月毎）を査定する。血漿RNAの減少が，単独薬または併用薬の効果を測定する方法として認められるようになっている。濃度の増加は，服薬指示が遵守されていないか，薬に対して耐性をもつ遺伝子突然変異体のHIVが出現したことを示す。 一部の専門家は，CD4数が比較的高くても（500/μL以上），血漿RNA量が測定可能（400コピー/mL以上）なレベルの患者に対しては治療を行うべきだとしている。疾患が進行していない患者（CD4が500以上）の場合，この集中的でコストのかかる治療アプローチを支持する証拠は確定的でない。全てのHIV感染段階において，ほとんどの患者で実証される急速なウイルスの産生とクリアランスがこのアプローチを支持している。
抗レトロウイルス薬：HIV感染の治療に使用される抗レトロウイルス薬を，分類別に一般名と略語で表163-3に挙げる。1998年中頃の時点でのFDAによる承認状況も記載されている。利用可能な薬物の4分類のうち3つは，HIV逆転写酵素を阻害することによって作用する。プロテアーゼ阻害薬はHIVプロテアーゼの活性を妨げる。
大多数の専門家が，HIV感染がどの段階でも，血漿1mL当たりのHIV RNAのコピー数が5000以上の患者は，2種類のヌクレオシド（例，ZVDおよび3TC［ラミブジン］），2種類のヌクレオシドとプロテアーゼ阻害薬（例，インジナビル），または2種類のヌクレオシドおよび非ヌクレオシド系逆転写酵素阻害薬（例，ネビラピン）などの併用療法による治療を受けるべきだとしている。薬の中には他の薬と相互作用して除去に影響を及ぼすものがあるが，場合によってはそれが役に立つ。例えば，2種類のプロテアーゼ阻害薬，サキナビルとリトナビルを併用すると，リトナビルはサキナビルの代謝を減らすことによって濃度を上げる働きをする。
もう1つの有用な相互作用は，薬に対して耐性をもつHIV遺伝子突然変異体の選択の防止または補償作用である。例えば，3TCを単独で投与すると，薬の存在下でもHIVが成長できるようにする突然変異をもつHIVを直ちに選択する。ZDV単独を数カ月続けると，多くの患者がZDVの抗ウイルス作用を打ち消す突然変異を発現する。しかし，3TCとZDVを併用すると，3TC耐性の突然変異がZDVに対するHIVの感受性を高めるため，その組み合わせはZDV耐性患者でもHIVの目覚ましい抑制を達成する。
併用によっていずれかの成分薬の排除が増減され，薬の濃度が高すぎる，あるいは低すぎてしまう場合，あるいは複合型の毒性がある場合には，併用は有害である。併用に関する情報が急速に蓄積されており，今後の選択に役立つであろう。
抗レトロウイルス薬の副作用は薬のタイプと投与量によって異なり，依然として患者と医師の双方にとって主要な問題である。 多くの副作用（例，ZDVでの頭痛）はしばしば時とともに軽くなるが，中には（例，ジダノシンでの腹痛）即座に対応する必要のある重大な問題（例，膵炎）を示しているものもある。副作用の中には，症状を引き起こす前に血液検査で検出できるものもあるため（例，貧血，膵炎，肝炎，グルコース不耐性），症状はもちろんのこと，血液および血清の生化学検査による定期的モニタリングが非常に重要である。最後に，治療の期間は不確定である。薬物は，その抗レトロウイルス効果が副作用とコストより勝っている間のみ服用すべきである。
薬を不適切な回数あるいは用量で患者に投与したり，患者が指示通りに薬を服用しないと，薬剤耐性が起こる可能性が高くなる。薬物の併用は耐性HIV突然変異体の選択を遅らせるが，ウイルス複製の完全な抑制が達成されない限り，通常は選択の防止にはならない。患者が服薬指示を遵守しているかどうかに注意し，血漿HIV RNAをモニタリングすることは，耐性菌株の選択を制限するのに役立つ。
現在のところ治療が不可能なHIVの一部の中枢神経系合併症（例，進行性多巣性白質脳症）は，一次免疫不全が補正されれば，抗レトロウイルス治療に反応する可能性がある。HIVによって誘発された認識機能不全が，抗レトロウイルス薬に対して反応したという報告がある。脳脊髄液中のHIV RNA量の測定は，HIV複製と脳に対する抗ウイルス治療効果を調べる手段になると思われるが，脳脊髄液RNAモニタリングの有用性はまだ実証されていない。
終末期のケア：有効性の高い併用療法が導入されたとはいえ，AIDSは依然として致死的な疾患である。疼痛や苦痛の緩和が治療の中心となる時期もあり，また患者がホスピスケアを選択する場合もある。
●薬物治療
HIV感染症の治療では、抗HIV薬3剤以上を併用した強力な多剤併用療法（HAART）を行う。初回治療では、 NNRTI＋NRTIあるいはPI＋NRTIのいずれかの組合せを選択する。近年服薬錠数の減少を目的としたNRTIの合剤が多くの患者に処方されるようになり、AZT／3TCやTDF／FTCと並んでABC／3TCも初回治療に用いられる傾向がある。

日笠らがアンケート調査にもとづいてわが国における抗HIV薬の使用頻度の推移を調査結果がある。
ヌクレオシド系逆転写酵素阻害剤（NRTI）の中では、依然として80％以上の症例に3TCが処方されていることがわかる。次に多いのがAZTで約半数の患者に投与されている。AZTと3TCの使用頻度は1999年以降大きな変化はないが、d4T、ddI、ddCの3剤は減少傾向にあり、ミトコンドリア障害等の長期副作用がその原因と推測される。逆に、ABC、TDF、FTCが増加傾向にある。一方、非ヌクレオシド系逆転写酵素阻害剤（NNRTI）とプロテアーゼ阻害剤（PI）の統計では、ここ数年増加していたEFV(41%)とLPV/r(23%)が横ばいとなり、かわってATV(18%)とRTV(17%)の使用が増えている。
RTVは他のPIと併用するboosterとしての使用である。NVP、NFV、IDV、SQVの使用頻度は、引き続き減少傾向が続いている。わが国で好んで選択されているHAARTメニューの半数以上の症例にTDF+3TC+EFVかTDF+3TC+ATV+RTVという1日1回投与の処方が選択されたことが明確に示されており、今後もこの傾向はますます強くなるものと予測される。第3位以降にはLPV/rを含むメニューが続いている。
●治療成績
内服しやすさ（服薬率の維持）により左右される傾向が高いが、服薬率95%以上でのHAART治療成功率は78.3%
●副作用
本邦でよく使用されているHAARTについて、(2003年4月1日～2004年3月31日 HIV感染症治療薬共同使用成績調査)からわかるように副作用頻度の合計は34～59％と高率であり、軽微な副作用も含めると「どの組み合わせを選んでも何らかの副作用が生じる」というのが実際のところである。しかしその内容をみると、薬剤ごとに頻度の高い副作用に一定の傾向が見られる。例えば、血液系の副作用はAZTを含んだ組み合わせで多く見られる。同様に、代謝系の副作用はd4TとPIを含む場合、精神・神経系の副作用はEFVを含む場合、消化器系の副作用はNFVを含む場合に多く見られる。皮膚の副作用はいずれの組み合わせでも見られる。
●薬剤耐性
抗HIV薬の治療の普及とともに薬剤耐性変異株による感染例が増加する懸念もあり､ 今後ともその動向には注意が必要である。 厚生労働省HIV検査法・検査体制研究班(2002)による日本人男性の未治療患者(121例)における耐性変異の検出率は6.6％であった。未治療のHIV感染者における薬剤耐性変異[IASR Vol.23 No.5 May 2002]
●予後
HIV感染者がAIDSを発症する，あるいは死亡する危険性は，CD4＋リンパ球数と血漿DNAの濃度を組み合わせることによって予測できる（前述の発生病理参照）。CD4＋数は日和見感染症に対する現在の易感染性に関する情報となり，血漿HIV RNA量は将来的なCD4＋数を予測する。抗レトロウイルス療法によって血漿RNA量を下げることにより，合併症と死亡の危険性が減少し，しばしばCD4＋リンパ球数が増える。日和見感染症は依然として，AIDS患者ほぼ全員の直接的な死因となっている。予防法の進歩によりニューモシスチス，トキソプラズマ，鳥型結核菌複合体，クリプトコッカスおよび他の日和見感染症の発生数が減少し，その結果，これらの日和見感染症が罹患率と死亡率に寄与する程度も低下した。これらの感染症や，範囲は限られているがカポジ肉腫に対する薬物治療方法が進歩したことにより，治療結果も改善されている。
抗レトロウイルス薬物の併用療法が導入されたことにより，AIDS患者の生存期間が2～3年と大幅に延びたが，効果の持続期間は様々で，まだ完全には定義されていない。強力な組み合わせで使用され，血漿ウイルス（RNA）量によってモニタリングされる新しい抗レトロウイルス薬は，HIV感染の全ての段階で患者の生存期間を延長できると期待されている。薬の効果は，患者が以前に抗レトロウイルス薬を使ったことがあるか，服薬指示をきちんと遵守するか，また感染の段階によって影響されるウイルスの抵抗性によって損なわれることがある。
