ホモシステインが冠動脈疾患のリスクファクターとして重要だ、という記述は、多くの文献や学術書の中に既定の事実として書かれていて、 国際動脈硬化学会[IAS]ガイドライン(2003)でもCVDのリスクファクター中[Emerging RiskFactors]の一つに"Elevated homocysteine"があげられている。
その上最近は、ホモシステイン低下作用を持つベタインらが健康食品として販売されるようになった。
今回は、ホモシステインがどういうものか、また実際はどうなのかを検証する。

・血漿ホモシステイン濃度の上昇は心血管系疾患と死亡のリスク増加に関連している。
・血漿ホモシステイン濃度の低下により、冠動脈疾患や脳卒中が減少したとの報告がある。
・そこで、血漿ホモシステイン濃度を低下すべきかどうかを検証した。
・葉酸の投与により血漿ホモシステイン濃度は低下するが、種々の患者に葉酸を投与した臨床試験では、心血管系に及ぼす作用について一定の見解は得られていない。
・現在のところ、ホモシステイン濃度の低下が有益だという確実な証拠はない。

臨床意義
先天性代謝異常症のホモシスチン尿症の診断および治療管理において有用である。ま
た，ホ モシスチン尿症患者に動脈硬化性血管障害が多発したこと，動物実験やinvitro
の研究によりホモシステインは血管内皮細胞を傷害することなどから，血漿ホモシステ
インの高値は動脈硬化性疾患，とくに脳梗塞の危険因子の一つとして示唆されている。

「ホモシスチン尿症」は、先天性アミノ酸代謝異常症のひとつで、血中にホモシステイ
ン、ホモシスチン、尿中にホモシスチンが増加する疾患。
新生児マススクリーニングの対象となっている。 約４０万人に１人。
放置した場合、進行性の知的障害や精神症状、骨格異常や水晶体亜脱臼などが見られ血
栓症で死亡することもある。
この原因には、シスタチオニン合成酵素欠損症、ビタミンB12代謝異常症、葉酸代謝異
常症等が知られているが、シスタチオニン合成酵素欠損症が最も発生頻度の高い原因。
シスタチオニン合成酵素は、ピリドキサール-5'-リン酸を補酵素とする酵素なので、ホ
モシスチン尿症ではビタミンB6の投与に反応するB6反応型と反応しないB6不応型とに分けられる。
ビタミンB6、葉酸などのビタミン療法が一般的。
またベタインを投与することで、ホモシステインをメチオニンに転換させる作用を利用
しホモシステインを減少させること、B6不応型の患者には、低メチオニン食+シスチン添
加、あるいはベタインを併用。

The American Heart Association has not yet called hyperhomocysteinemia (hi"per-ho"mo-sis-TE'in-E'me-ah) (high homocysteine level in the blood) a major risk factor for cardiovascular disease. We don't recommend widespread use of folic acid and B vitamin supplements to reduce the risk of heart disease and stroke. We advise a healthy, balanced diet that includes at least five servings of fruits and vegetables a day. For folic acid, the recommended daily value is 400 micrograms (mcg). Citrus fruits, tomatoes, vegetables and grain products are good sources. Since January 1998, wheat flour has been fortified with folic acid to add an estimated 100 micrograms per day to the average diet. Supplements should only be used when the diet doesn't provide enough.
